Many difficult treatment decisions have to be made by individuals experiencing brain injury, their families, case managers and other professionals. Community NeuroRehab’s (CNR) referral specialists and admission team members provide education and support to these individuals, working to ensure they have accurate information to make informed decisions about their rehabilitation needs.

CNR offers community-based residential rehabilitation centers, intermittent in-home rehabilitation, or other service options to enhance your current treatment experience. Our referral specialists are credentialed in brain injury and will help you get the answers you need. They will support you through the inquiry, referral and admission processes and if at any time it is determined that CNR is not the appropriate fit for the current need, our specialists will assist you in finding what you need.
